Insurance
We are in network with most Blue Cross and Blue Shield of Texas plans, Aetna, Ambetter from Superior HealthPlan, and Medicare.
Plans we currently accept:
- Blue Cross and Blue Shield of Texas (most plans)
- Aetna
- Ambetter from Superior HealthPlan
- Medicare
We bill in-network plans directly. Before your first visit, we ask for your insurance information and confirm your benefits, so you know what your plan covers and what your visit will cost before you are seen. Coverage varies by plan, and some plans require a referral or prior authorization for certain services.
If your plan is not listed, or you prefer not to use insurance, you are welcome to be seen as a self-pay patient at the rates below. We do not provide superbills for out-of-network reimbursement.

What visits cost
Self-pay fees
Our fees are published so that you know the cost before you schedule. The rates below apply to self-pay visits; with an in-network plan, your cost is set by your plan's deductible, copay, or coinsurance.

The fee is the same whether a follow-up appointment is conducted in the office or by video.
How to pay
Payment
Payment is due at the time of the visit. For in-network plans, this means your copay, coinsurance, or deductible amount; for self-pay visits, the full fee. We accept credit and debit cards, HSA and FSA cards, and checks.
Rescheduling
Rescheduling
Appointments may be cancelled or rescheduled online up to 24 hours before the appointment time. Inside 24 hours, the online system will not permit a change, and you will need to call the office at 972-332-5922. Please call us in that situation rather than not attending.

Billing
Questions about a bill
For questions about your bill, please call 972-332-5922, request assistance in writing to our office address, by email to [email protected], or through the patient portal.
We will acknowledge your question in writing within 5 business days and provide a written answer, in plain language, within 30 days.
You may also request the following at any time, at no charge:
- A written estimate of expected charges. Estimates are provided on the timeline described on the Good Faith Estimate page.
- An itemized bill, on request made within one year of the visit, provided within 10 business days. The first two copies are provided at no charge.
- A plain-language explanation of any charge.
- A refund of any overpayment, issued within 30 days of the determination that an overpayment was made.
